Sharp Football Academy

Liability Waiver

1. ASSUMPTION OF RISK: I acknowledge that participation in the Sharp Performance Top Prospect & Media Day event involves physical activities that carry inherent risks of injury, including but not limited to sprains, strains, fractures, concussions, heat-related illness, and other physical injuries. I voluntarily assume all such risks, both known and unknown, and accept personal responsibility for any injury, disability, death, or loss sustained as a result of participation.

2. RELEASE OF LIABILITY: I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, and forever discharge Sharp Performance LLC, Sharp Football Academy, Kansas Wesleyan University, and their respective officers, directors, employees, agents, volunteers, sponsors, and affiliates from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained during or in connection with this event.

3. MEDICAL FITNESS: I certify that the participant is physically fit and has no medical condition that would prevent safe participation. I understand that Sharp Performance does not provide medical personnel on-site and that I am responsible for disclosing any relevant health conditions. I acknowledge that participation is at my own risk.

4. EMERGENCY MEDICAL TREATMENT: In the event of an emergency, I authorize Sharp Performance and its staff to seek and consent to emergency medical treatment for the participant, including but not limited to first aid, CPR, and transport to a medical facility. I agree to be responsible for all associated medical costs.

5. RULES AND SAFETY: I agree that the participant will follow all rules, instructions, and safety guidelines provided by event staff. I understand that failure to comply may result in removal from the event without refund. Sharp Performance reserves the right to modify, delay, or cancel the event due to weather, safety concerns, or other factors beyond its control.

6. MINOR PARTICIPANTS: If the participant is under 18, I, as parent or legal guardian, consent to their participation and agree to all terms of this waiver on their behalf. I understand that this waiver is legally binding and that I am giving up substantial legal rights by signing.

Media Release

I hereby grant Sharp Performance LLC, Sharp Football Academy, and their authorized representatives the irrevocable right and permission to photograph, video record, and otherwise capture the participant's likeness, image, voice, and athletic performance during the Top Prospect & Media Day event.

I consent to the use of such materials for any lawful purpose, including but not limited to: promotional materials, social media content, website content, recruiting profiles, highlight videos, media distribution, and marketing — without compensation or further approval required.

I further authorize Sharp Performance to share the participant's recruiting information — including name, school, graduation year, position, athletic measurements, performance data, contact information, and film — with college coaching staffs, recruiting services, media outlets, and other third parties for the purpose of facilitating recruiting exposure and opportunities.

This release is binding and shall remain in effect in perpetuity unless revoked in writing.
